Discoid Eczema

Discoid eczema, also known as nummular dermatitis, is a skin condition which can occur at any age.

The cause of discoid eczema is unknown.

• Minor skin injury such as an insect bite or a burn may start it off.
• It may be termed "varicose eczema" when it runs along the leg veins and result in gravitational eczema.
• Hypersensitivity to bacteria on the skin results in crusted patches.
• Dry skin in the winter months can cause dry non-itchy round patches.

It can affect any part of the body particularly the lower leg. One or many patches appear, and may persist for weeks or months. The majority of patches are round or oval, hence the name "discoid" or "nummular" dermatitis, which means coin or disc-shaped dermatitis. They can be several centimetres across, or as small as two millimetres. The skin between the patches is usually normal, but may be dry and irritable. Discoid eczema may be extremely itchy, or scarcely noticeable. When the patches clear, they may leave marks for some weeks of months which are darker or sometimes paler than the normal skin tone. Sometimes patch testing is arranged to see whether there could be a contact allergy responsible for the dermatitis. In most cases no specific allergy can be found. Discoid eczema does not run in families, and unlike atopic dermatitis, it is not associated with asthma. It does not result from food allergy. It is not infectious to other people, although it sometimes becomes secondarily infected by bacteria.

The terms eczema and dermatitis describe a reaction pattern of skin disorders with a variety of common characteristics. The acute disease typically is characterized by inflammation, redness, swelling, and itching, as well as some blistering and oozing. Skin biopsies show inflammatory cells and swelling. When dermatitis becomes subacute, the intense reaction becomes milder, and the blisters begin to heal. Crusts and scale are common at this time. Chronic dermatitis is identified by thickened, leathery skin with excess ridges, as well as dark and dull skin. Under the microscope, the outermost (epidermal) skin layer is seen to proliferate and become elongated. There is a wide range of dermatitis types: Atopic dermatitis is an inherited condition characterized by dry, sensitive, itchy skin. Infants and children are most affected, and the disorder can continue into adulthood.   Contact dermatitis is caused by an irritation or an allergy and occurs on areas of the skin that have come in contact with the identified substance. These reactions can be sudden and severe, such as with poison oak or poison ivy, or more insidious and chronic, such as from repeated exposure to harsh soaps or chemicals.   Seborrheic dermatitis is a fairly common condition seen mostly in adults and has a tendency to affect men more than women. The disease has a natural waxing and waning course. The cause is unknown.   Nummular dermatitis usually appears as stubborn, coin-shaped eczema plaques on the legs. It may be related to skin dryness, and moisturizers seem to help relieve the condition. FAQs Q: My child has sensitive skin and gets rashes easily with bubble baths and wool clothing. What can I do to prevent this? A: Your child may have mild atopic dermatitis. It occurs most frequently in families with a history of eczema, allergies, and asthma. This disorder is characterized by dry, sensitive, itchy skin. Mild soaps, cotton clothing, and moisturizers help minimize flare-ups. In severe cases, prescription medications may be necessary. Q: I was diagnosed with poison ivy, although I have not been exposed for at least 2 weeks. Is this possible? A: Yes. It can take up to 3 weeks after exposure for the typical allergic reaction to poison ivy to appear. Q: I was treated for severe poison oak with prednisone for a week. When I stopped, the rash and itching returned. What went wrong? A: The allergic reaction that follows contact with poison oak, ivy, or sumac can last up to 3 weeks, despite bathing and laundering the contaminated clothing. For severe outbreaks, oral corticosteroids are prescribed. Treatment may need to continue for 3 weeks, until the body's internal reaction has subsided. Q: My child has atopic dermatitis. Should he avoid certain foods? A: Food allergies have been linked to atopic dermatitis in only about 10 percent to 15 percent of cases. The most common sensitivities are to eggs, milk, soy, wheat, nuts, and fish. If you suspect a food allergy, your pediatrician or dermatologist can help you eliminate various items from your child's diet to confirm your suspicion. Allergy tests are infrequently done. Q: My seborrheic dermatitis flares up when I am under stress. Is this common? A: Yes, some people experience flare-ups during stressful times. In addition to getting adequate rest and nutrition, you may want to switch to dandruff shampoos and use a prescribed antifungal cream. Mild cortisone creams help minimize the flare-ups but should not be used for long periods of time. Back to Top

Nummular Dermatitis

I. Definition:

Nummular eczematous dermatitis (nummular eczema or nummular dermatitis) is a name given to a stubborn, sometimes itchy rash that forms coin-shaped patches on the skin. The lesions as they get older may clear in the center resembling ring worm or fungus. The condition tends to be chronic, with periods of quiescence and exacerbation.

II. Cause:

• The cause is unknown. It is more common in the winter.
• Nummular eczema is frequently associated with dry skin.
• Wool, soaps and frequent bathing (more than once a day) often worsen the condition.
• People with eczema have skin that is dry and easily irritated by soap, detergents, and rough wool clothing. Clothes washed or dried with liquid or sheet fabric softeners such as Cling, may also irritate the skin. Hot and cold weather often aggravates eczema. Certain allergies may worsen eczema, but they don't cause it.

III. Treatment

• There is no cure. However, there are effective ways of controlling it.
• Vaseline, tar and cortisone compounds applied to the skin are the best medicines for controlling nummular eczema. Most cortisone salves can be used safely for years. When large areas of the body are treated with strong cortisone preparations, periodic medical check ups are necessary. Strong cortisones shouldn't be applied to the face, armpits, groin, or rectal area.
• When using cortisone creams or lotion always remember to use a little and massage it in well.
• For stubborn nummular eczema, salves containing coal tar can be helpful. However, it smells and stains the clothing.
• Keep the skin lubricated. Use a bath oil or Aveeno powder in the tub. Do not take more than one bath or shower a day. Use lukewarm water, not hot. Hot water dries out the skin. When toweling dry don't rub. Blot the skin so there is still some water left on the skin. Then apply a moisturizer to all of the skin such as Vaseline, Lubriderm, or Moisturel.
• Soap irritates and dries the skin, so keep it away from your eczema. When bathing limit the use of soap to your face, armpits, genital area, and feet. Use Cetaphil soap, Oil of Olay, Dove or Basis. If hands are washed frequently, dry carefully then apply a little Vaseline.
• Avoid contact with wool or rough clothing. Cotton clothes, (100%) are best.
• When laundering the clothes, use no fabric softener, Cling or dryer sheets. Wash the clothes using dye free, fragrance free detergents or Ivory Snow.
• Avoid make-ups, perfumes, etc. that may cause itching or irritation.
• It is possible to find a treatment routine that lets you control your nummular eczema. Usually with cortisone creams or ointments and a lot of Vaseline.
• If your nummular eczema worsens and you can't control it, return to the office so we can adjust your medications.
