Researchers Hope to Develop New Method of Screening for Heart Attack Risk

B O S T O N, July 31 — Despite all the attention paid to the dangers of narrowed, clogged arteries, doctors have recently come to believe a new culprit is responsible for the vast majority of the 1.1 million heart attacks that occur annually in the United States.
    
Heart researchers are focusing on “vulnerable plaques,” buildups of fatty tissue in blood vessels that can unexpectedly rupture, causing as many as two-thirds of all heart attacks.
     Now, researchers at Mt. Sinai School of Medicine in New York City are trying to replace current, imperfect methods of detecting these oncoming “plaque attacks” with a newer, non-invasive procedure using magnetic resolution imaging (MRI) technology.
     The researchers used MRI machinery to take a close-up snapshot of 13 patients’ coronary arteries, looking for “vulnerable” plaques which build up inside tiny blood vessel walls.

[Karl Note: The use of the phrase "build up" above gives away the false understanding they have, and gives you yet another reason why "the public" believe false information about "plaque."  That is virtually all they get -- they get false data.]
     When these plaques suddenly rupture, they can cause clots that plug up a blood vessel, preventing blood flow to the heart and resulting in unpredictable — and often lethal — heart attacks.
     The research, led by head investigator Zahi Fayad, Ph.D., was reported today in Circulation, the journal of the American Heart Association.
     “The findings of this promise a much-needed, non-invasive method to identify “vulnerable” plaques,” says Dr. Claude Lenfant, director of the National Heart, Lung and Blood Institute at the National Institutes of Health, which funded the study. “The new MRI techniques may become an important tool to help physicians identify those individuals who are most likely to suffer a heart attack.”

[Karl Note:  Dr. Lenfant, of course, is one of the TROLLS who is responsible for much of the false information about heart disease in America.  However, he doesn't object to the concept of "vulnerable plaque" because this concept has not much paid attention to HOW the plaque was created, or if it does, it simply indicates the same old false problem of "eating eggs and butter."]

Beyond Treadmill Tests
Current technologies, such as “treadmill stress tests,” which monitor patients’ hearts while they exert themselves, and cardiac catheterization, which threads a needle through a patient’s artery, only measure restricted blood flow and can completely miss the presence of these dangerous plaques.
     “That’s why it’s possible to have a treadmill test that looks fine, and suddenly die of heart disease,” explains Dr. James Muller, director of clinical research in cardiology at the Massachusetts General Hospital in Boston.
     Meanwhile, the popular “electron-beam computed tomography,” (EBCT), a competing, non-invasive test costing about $500, is still not universally recommended as a screening method by the American College of Cardiology. This test looks for calcium deposits in the arteries, which are an indication for plaques, but may not detect the actual plaques themselves.
     Doctors say the battle between the two techniques is still unresolved.
     “Vulnerable plaque is the leading cause of death in this country, and there are technologies that could find them,” Muller says. “This is a problem that has a technological solution, but it is not clear yet which technology will solve it.”
     Fayad claims that the coronary MRI technique is the only non-invasive procedure that examines the quality of their vessel walls. “The CT does not cover the whole picture,” he says. “People need to see the [vessel] wall.”

At What Cost?
But many doctors say that MRI may not be “the” final answer due to costs and complexities.
     “The jury is still out as to what technique will do better,” says Richard White, head of cardiovascular imaging at the Cleveland Clinic. “The primary appeal of the MRI is that is has no X-ray exposure. But the CT will be cheaper across the board.”
     “MRI is a costly technology that requires very sophisticated technical support, as well as a cooperative patient who has no metal [pacemakers or valves] in their body, no claustrophobia, and no abnormal heart rhythm,” says Dr. Randy Martin, professor of medicine at Emory University in Atlanta. “The real question is, could this technique be widely applied? It may not become universally available.”
     At the very least, says Dr. Peter Libby, chief of cardiovascular medicine at Brigham and Women’s Hospital in Boston, the coronary MRI will be an “invaluable research tool” for learning more about “vulnerable” plaques and assessing interventions.
     Others in the field are busy developing an array of more invasive methods to inspect the plaques, including blood tests, internal MRIs, ultrasounds, heat-sensors and visual scopes.

Predicting Risk
The competing techniques will be presented at a conference in New York City this September organized by Dr. Valentin Fuster, director of the Cardiovascular Institute at Mt. Sinai School of Medicine. “We’re going to discuss what are all the methods available, invasive and non-invasive,” he says.
     “It’s a massive, national effort to develop the technology,” says Muller, who has founded a start-up firm himself, called Infraredx, that hopes to use an infrared catheter to detect whether a plaque is full of dangerous cholesterol or more benign collagen.
     Doctors have found that cholesterol-lowering drugs seem to stabilize these vulnerable plaques, reducing inflammation and allowing them to be reabsorbed into the body.
     But someday, Muller says, standard practice may include screening the population for vulnerable plaques during an office visit, and, instead of prescribing medication, operating preventatively on the dangerous plaques to prevent a future unexpected rupture.
     “We accept heart attacks as unpredictable things,” Muller says, “and we shouldn’t.”
